BCP (Band Credit Point) Withdrawal / Deposit Form

Download this form to complete and use to withdraw funds from your student’s BCP account. You may also use this form to make deposits to your student’s account. Must be signed by a parent or guardian.

NOTE: The BCP Transaction Authorization Form MUST be used to pay for expenses with BCP funds . . . verbal or E-Mail requests can not be processed in order to ensure the proper credit is given and records are maintained.

Parents or students may use the same BCP Transaction Authorization Form to make deposits into their BCP account as well. Simply place a BCP Transaction Form along with your check in the Purple Box.

At the end of the school year, any remaining points in the account are rolled over into the next school year. Parents of graduating seniors will have the choice of donating their unused points into the Alice Masone scholarship, or transferring the amount to a younger sibling in the band or someone else of their choosing. If a sibling will be entering the band from middle school within two years, the money can be held in an account for them. If the parent does not designate the points to one of these selections, the points will be placed in the Operating fund of the Band Boosters.

Band Student Travel Waiver

It is Rock Hill School District #3 policy to only allow a student to leave a school activity (away ballgame, contest, field trip, concert, band camp, etc.) with their parent or an adult legal guardian, not other family members, friends, students or neighbors. To avoid any confusion, any student wishing to leave an away event (with their parent or guardian) must have this form filled out and signed by parent or guardian The form must be handed in to Mr. Wells, or the chaperone in charge on their assigned bus.

Concessions Stands
Permission Form

Northwestern Band Boosters, Inc. currently provides opportunities for you and your student to work service fundraisers to earn credit for band fees that are incurred during the year and for elective spring trips the band students and families choose to participate in.

NHS Booster Club parents will supervise all concession booths at the above-mentioned stadiums. Some of these stands do sell alcoholic beverages, but Northwestern Band policy states students are not allowed to physically handle these beverages. Each student working these concession fundraisers will abide by this policy and must have a permission slip signed by both the student and the parent in order to work.

Private Lessons
Request Form

Private lessons at Northwestern is an opportunity for students to receive individual instruction on their instrument from an accomplished musician. Through this program, students will improve such basic skills as tone production, technical facility, and sight reading skills. The student will also gain a deeper appreciation of the musical nuances needed to enhance the possibility of college music scholarships which are available to students who are not music majors at many colleges and universities.

The private lessons are scheduled once a week in the Northwestern High School band room. The lessons will be 30 minutes long and are taught by Winthrop University music students and local professional musicians. The lesson fees range from $10 to $20 a lesson depending on the experience of the teacher. Most lessons are scheduled between 4:00 and 7:00 p.m. and the day will be determined by the availability of the teacher. We have contacted teachers who are both qualified and dependable and they expect that the students who enroll in the private lesson program to be on time and prepared for their lessons.

COMPLETE THIS FORM ONLY IF YOU ARE SERIOUS ABOUT PRIVATE LESSONS
